POSITION TITLE: Parent Empowerment Case Manager

JOB TITLE:  Parent Empowerment Case Manager for Catholic Charities-Diocese of Lubbock

POSITION TYPE:  Full-time (may require some evenings and weekends)

ABOUT THE PARENT EMPOWERMENT PROGRAM:

The Parent Empowerment Program at Catholic Charities is a rapidly growing, high-energy program dedicated to breaking the cycle of generational poverty with education. The program assists single parents and low-income parents get their education in several fields, including nursing school, vocational programs, and traditional colleges and universities. Parent Empowerment Program graduates use their education, certifications, or degrees to find gainful, fulfilling employment and support their families, breaking the poverty cycle.

Key Responsibilities

Case Management:

  • Identify and screen participants for eligibility, interview and enroll those that meet eligibility requirements.
  • Assist with goal setting and proactive activities; including formulating plans for service/goals and monitor progress each month.
  • Keep accurate and detailed records (case note for every assistance or monthly)
    Recruit eligible persons for assistance.
  • Email program participants regarding mandatory workshops and keep attendance records.

Referrals/Resources:

  • Maintain knowledge of funding sources (internal and external) and eligibility criteria/limitations
  • Work closely with CC staff to refer participants for counseling, emergency services, etc.

Administrative Tasks:

  • Return phone calls and emails.
  • Open/Close Online intakes as needed.
  • Properly fill out required forms and grant reports.
  • Collect invoices from participants.
  • Certify invoices for payment (based on grant eligibility requirements)
  • Log in payments and enter assistance/case notes within 24 hours in database.
  • Work closely with department director in day-to-day grant operation.

QUALIFICATIONS

  • Associate’s Degree, or Bachelor’s degree in social work or in a related field OR 2–3 years of related work experience
  • Strong written and verbal communication skills
  • Proficiency in case management, working with LMI participants.
  • Basic knowledge of existing resources
  • Excellent organizational and multitasking abilities; time management skills, able to meet strict deadlines.
  • Detail-oriented with strong analytical skills, ability to work independently, as well as with others in collaborative environment.
  • Proficiency in Microsoft Office Suite and ability to learn other software as needed.
  • Passion for the organization’s mission
